Aim: Triple-A Syndrome (TAS) is a rare autosomal recessive disorder characterized by three cardinal symptoms: alacrimia, achalasia and adrenal insufficiency due to ACTH insensitivity. The disease is caused by mutation in the AAAS gene on chromosome 12q13. Mutations in AAAS were identified in more than 90% of individuals and families with TAS. Co-existence of malign insulinoma and diabetes mellitus

Insulinomas are rare neuroendocrine tumors (NETs) of the pancreas with an incidence of four per 1 million persons per year. The co-existence of diabetes mellitus (DM) and insulinoma is very rare. We report a case of 73 year old women with malignant insulinoma and type 2 DM.
